Instructions

To download the form, just click on the relevant link depending on your level within your LA/organisation.
Once you have downloaded the form, you can either fill it in electronically or print it out and write your answers in.
If you choose to fill it in electronically, please select ‘Save As’ from the file menu and save it in a suitable folder where you will be able to find it easily. Please then attach it to an email and send the email to events@virtualstaffcollege.co.uk
If you choose to fill it in by hand, please fax the whole form back to 0161 838 5756

Please note that 2nd tier managers (often called Assistant Directors) refer to those who report to the Director of Children’s Services.

3rd tier managers (often called Heads of Service) refer to those who report to Assistant Directors.
